Welcome to a New Sports Season!

As you confirm your child's registration or are registering your child for the first time in Final Forms be sure the following are complete:

Electronic signatures have been completed by parents and student.
All addresses, emails, and cell numbers are correct.
IMPORTANT:
- Download and print the IHSAA physical form for middle school and high school students.
- Once the doctor has completed and signed the physical form, upload the completed form to Final Forms.
- New physicals are to be completed after April 1st, 2024 and prior to your child's first practice.
- A student will be eligible once all details (electronic signatures and physicals) are complete.

The Athletic Department will confirm eligibility.
